ALL Fall through Spring Semesters

All SF College ASN/PN/NA nursing students are required to obtain “official” documentation showing proof of the 2024-2025 flu vaccination and detailed information about the vaccine from the provider of the vaccine (person who administers your flu vaccination).

The detailed information must include the following:

Name of provider (who gave you the injection)

Name of student/faculty

Date of administration

Name of vaccine

Manufacturer of vaccine

Lot number

Expiration date of the flu vaccine.

If any of the required information (listed above) is missing, you will be re-directed to the provider to obtain this information prior to being cleared for clinical.

All students must upload official flu documentation in the provided drop box located in their canvas course as indicated. Once I received you will be issued a flu sticker. Once you receive your flu sticker you must apply the sticker to your SF College picture ID at this time (Please do not lose your flu stickers as I am only given one per student and cannot give you another one). Your SFC College picture ID badge will be worn at all times when you are in clinicals with the stickers visible.

Please contact Professor Schwab if you cannot take the vaccine due to medical contraindications at this time. You will submit a medical letter from your PCP in this rare circumstance for the current Flu season.
